13 to 18 Years of Age
Age: 13-18 years
Frequency of Visits:
1 visit every other year
Examination to Include:
- Interval history, Height & weight, blood pressure
- Scoliosis screening
- Testicular exam or Breast exam
- Hematocrit/Hemoglobin & Urinalysis - once at 13-18 years
- Pap Smear - annually if sexually active
- Serum Cholesterol/HDL - once every 5 years, if there's a family history of early heart disease or hyperlipidemia
Immunizations & Counseling:
- Tetanus-diphtheria (Td) booster: once at 11-16 years
- Hepatitis B (3 doses) & Varicella (2 doses) - if not previously immunized
- Breast self-examination: teach to perform
- Testicular self-examination: teach to perform
- Alcohol, Tobacco, Substance Abuse: identify risk & counsel
- Sexually Transmitted Diseases: identify risk & counsel
