Age: 19-39 years
Frequency of Visits:
1 visit every 1-3 years
Examination to Include:
- Interval history, Height at initial exam or once after age 22, weight & blood pressure
- Testicular exam or Breast exam
- Serum Chloesterol/HDL - once every 5 years
- Pap Smear - annually
- Mammography - once at 35-39 years
Immunizations & Counseling:
- Tetanus-diphtheria (Td) booster: once every 10 years
- MMR - if born after 1956 and less than 2 previous doses
- Varicella (2 doses) - if no history of chicken pox nor previous immunization
- Breast self-examination: teach to perform
- Testicular self-examination: teach to perform
- Alcohol, Tobacco, Substance Abuse: identify risk & counsel
- Sexually Transmitted Diseases: identify risk & counsel
