19 to 39 Years of Age

Age: 19-39 years

Frequency of Visits:
1 visit every 1-3 years

Examination to Include:

• Interval history, Height at initial exam or once after age 22, weight & blood pressure
• Testicular exam or Breast exam
• Serum Chloesterol/HDL - once every 5 years
• Pap Smear - annually
• Mammography - once at 35-39 years

Immunizations & Counseling:

• Tetanus-diphtheria (Td) booster: once every 10 years
• MMR - if born after 1956 and less than 2 previous doses
• Varicella (2 doses) - if no history of chicken pox nor previous immunization
• Breast self-examination: teach to perform
• Testicular self-examination: teach to perform
• Alcohol, Tobacco, Substance Abuse: identify risk & counsel
• Sexually Transmitted Diseases: identify risk & counsel
