2 to 6 Years of Age

Age: 2-6 years

Frequency of Visits:
1 visit per year

Examination to Include:

• Interval history, Height & weight, blood pressure
• Hematocrit/Hemoglobin & Urinalysis - once at 2-6 years
• Vision & Hearing screening - at least once at 2-6 years
• Serum Cholesterol/HDL - once at 2-6 years, if there's a family history of early heart disease or hyperlipidemia

Immunizations & Counseling:

• DTP (or DTaP): once at 4-6 years
• OPV (polio): once at 4-6 years
• Hepatitis B (3 doses) & Varicella (1 dose) - if not immunized in infancy
