7 to 12 Years of Age

Age: 7-12 years

Frequency of Visits:
1 visit every other year

Examination to Include:

• Interval history, Height & weight, blood pressure
• Hematocrit/Hemoglobin & Urinalysis - once at 7-12 years
• Serum Cholesterol/HDL - once every 5 years, if there's a family history of early heart disease or hyperlipidemia
• Scoliosis screening

Immunizations & Counseling:

• MMR once (second dose) at or before 12 years
• Hepatitis B (3 doses) & Varicella (1 dose) - if not previously immunized
