Frequency of Visits:
1st Year: 6 Visits
2nd Year: 2 Visits

Examination to Include:

• Interval history & developmental assessment
• Height, weight & head circumference
• Hematocrit/Hemoglobin - once during birth to 6 years
• TB skin test - once every 5 years

Immunizations & Counseling:

• DTP: 2, 4, 6 and 15-18 months
• OPV (polio): 2, 4 and 15-18 months
• MMR: Once at 12-15 months
• HIB (meningtitis) 2, 4, 6 and 15-18 months
• Hepatitis B: 3 doses during first 18 months
• Varicella: once at 12 months
• Assessment of lead risk
